Description
The use of kratom and kratom-derived products has expanded rapidly across the United States, with many individuals perceiving these substances as safe, natural alternatives for pain management, mood enhancement, and opioid withdrawal. More recently, highly concentrated and semi-synthetic products containing 7-hydroxymitragynine (7-OH), the primary opioid-active alkaloid of kratom, have emerged in commercial markets and online retailers. These products possess significant opioid receptor activity and are increasingly associated with tolerance, dependence, withdrawal, and other opioid-like adverse effects. Addiction medicine clinicians are now encountering patients using 7-OH products in detoxification, residential, outpatient, emergency, and primary care settings, often without adequate toxicology testing or standardized treatment guidance. This presentation will provide an evidence-based review of kratom and 7-OH pharmacology, emerging clinical trends, diagnostic challenges, and practical treatment strategies, including the use of medications for opioid use disorder when clinically indicated.

Dr. Kirby previously served as President of the Tennessee Society of Addiction Medicine and now holds a national leadership role as Treasurer on ASAM’s Executive Board, where he also chairs the Finance Committee. Board-certified in both Addiction Medicine and Family Medicine, his work spans clinical care, healthcare economics, and policy, with a focus on expanding access, improving quality, and shaping the future of addiction treatment. He also contributes to national guidelines and policy through multiple ASAM committees and currently serves as a Health Policy Fellow with AACOM, reflecting his ongoing commitment to driving meaningful change in healthcare systems and patient outcomes.
