Psychedelic-assisted therapy has re-emerged as one of the most active areas of investigation in addiction medicine. This 60-minute presentation provides a comprehensive, evidence-based update on the clinical trial landscape for psilocybin, MDMA, ibogaine, and DMT/ayahuasca in the treatment of substance use disorders. Beginning with a brief historical overview and mechanistic foundations, the talk reviews landmark and recent trials — including the Bogenschutz et al. (2022) psilocybin-for-alcohol RCT, the Johnson et al. (2026) psilocybin-for-smoking RCT, the Hendricks et al. (2026) psilocybin-for-cocaine RCT, and the MAPS MDMA-assisted therapy program — before addressing cross-cutting issues critical to addiction medicine practice: methodological limitations such as functional unblinding, safety profiles across compounds, regulatory developments at the federal and state level, equity in research and access, and informed consent challenges unique to psychedelic medicine. Attendees will leave with a clear evidence hierarchy for each agent and a practical framework for evaluating the field as it evolves toward potential clinical implementation.
